Registered Nurse (RN) - Full Time/Per Visit, - Home Health
Frontpoint Health is looking for a full‑time Registered Nurse (RN) to provide home‑health care on a per‑visit basis.

Description:
The RN will perform a variety of duties to help the company thrive and manage continuity of care. The candidate must maintain 32 productivity points weekly to qualify and participate in the compensated on‑call program. The RN will see all visits assigned within the scope of practice of their license, which includes OASIS visits, skilled nursing visits, and supervised visits. The RN will administer skilled nursing care, coordinate care with the interdisciplinary team, patients, families, and referring agencies, and participate in the coordination of care.
Job Responsibilities:
Case management and supervised visits.
Perform skilled and comprehensive assessment of the patient during the initial admission.
Assess and chart observations of the patient’s condition at each visit (vital signs).
Provide skilled nursing care as needed in accordance with the plan of care (administer medication, clean/dress wounds, assess healing progress).
Provide proper patient education for home‑health care procedures and strategies.
Maintain clinical records: visit notes within 24 hours, OASIS within 48 hours, medication profile, orders/plan submitted timely.
Maintain confidentiality of patient PHI through HIPAA law.
Provide palliative care to keep patient comfortable.
Qualifications:
Current Texas State license as a Registered Nurse.
2+ years experience as an RN in Home Health.
Current CPR certification.
Reliable transportation.
Texas Driver’s License and Auto Liability Insurance.
Current TB Skin Test.
EMR experience (Kinsser preferred).
