Responsibilities
Deliver exceptional care to the client in their home.
Help execute a comprehensive personalized weekly schedule and care plan.
Provide skilled nursing services per the client’s plan of care within scope of practice.
Counsel clients and family to meet nursing and related needs.
Meet all documentation requirements according to agency policy.
Coordinate services through case conferences, case management, team meetings, and staff meetings as indicated.
Perform administrative functions such as documentation of task completion and reporting changes in the client’s physical condition, mental capability, or behavior.
Qualifications
Licensed Practical Nurse (LPN) license required.
BLS certification required.
Experience with peripheral IV placement, access ports, and PICC lines required.
Sound judgment and calm demeanor in diverse situations.
Basic computer skills.
Proof of eligibility to work in the U.S.
Licensed Practical Nurses must comply with state background screening requirements and must be eligible to work in the U.S. Compensation, benefits, and bonuses vary by state; please ask for details at interview.
