As an
In‑House Nurse Practitioner
specializing in Infectious Disease, you will be the primary provider helping your facilities clinical expertise in their Infectious Disease programs by participating in appropriate resident's plan of care. You will have the opportunity to build strong, lasting relationships with residents, their families, facility staff, and providers, while being an integral part of the clinical leadership team.
At Aviata Health Group, we focus on preventing critical health issues before they arise, delivering proactive care to residents in our Skilled Nursing and Long‑Term Care facilities.

Job Requirements
Active Florida Nurse Practitioner License (unencumbered).
Master's Degree in Nursing from an accredited program.
Strong clinical background in Infectious disease.
Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long‑term care, hospice, palliative care, or similar settings.
Ability to engage and communicate effectively with elderly and chronically ill patients and their families.
Ability to meet a minimum of 12 patient visits per day, with bonuses for surpassing this target.
Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.
Key Responsibilities
Perform comprehensive physical examinations and evaluate changes in patient conditions.
Prescribe medications and treatments as needed.
Develop patient care policies and facilitate referrals to specialists.
Coordinate advanced care planning and discharge visits.
Ensure timely communication and documentation of patient assessments, treatment plans, and follow‑ups.
Support the physician‑patient relationship and address gaps in care.
Salary & Compensation
Annual Pay: $105,000 - $130,000 based on experience.
Monthly Bonus Opportunities: Performance‑based bonuses.
