Position Summary
We are seeking a licensed Physical Therapist to assess, plan, and implement individualized physical therapy treatment programs in accordance with federal, state, and local regulations. This role is responsible for delivering high-quality, patient-centered care, maintaining accurate and timely clinical documentation, and collaborating with an interdisciplinary healthcare team. The Physical Therapist will practice in compliance with professional and ethical standards established by the Oregon Board of Physical Therapy. Formal mentorship is available for new graduates or therapists transitioning into outpatient therapy.

Provide supervision to Physical Therapist Assistants, aides, and students in collaboration with other therapists, as appropriate
Key Responsibilities
Assess patient conditions, establish treatment goals collaboratively with patients, and develop individualized treatment plans
Accurately code physical therapy diagnoses and procedures
Verify required authorizations prior to evaluations or treatment sessions
Document evaluations, treatments, and patient progress in the medical record
Develop competency in the E-ClinicalWorks EMR system
Supervise Physical Therapist Assistants to ensure treatment plans are followed appropriately
Operate therapeutic equipment and modalities, including exercise equipment, manual therapy tools, and soft tissue mobilization devices
Collaborate with physicians and other healthcare professionals to support coordinated, interdisciplinary patient care
Communicate with referring providers to update and revise treatment plans based on patient progress and overall health goals
Evaluate, fit, and adjust prosthetic and orthotic devices; recommend modifications as needed
Educate patients on home exercise programs and the use of assistive devices (e.g., crutches, canes, prostheses)
Adhere to CMS reporting standards, including MIPS-related requirements
Follow clinic policies and procedures as outlined in the employee manual
Serve as a clinical resource for patients, families, physicians, and staff
Complete department-specific training and maintain required competencies
Qualifications
Doctoral, Masters, or Bachelors degree in Physical Therapy from a CAPTE-accredited program
Active Oregon Physical Therapy license or eligibility to obtain one
Current BLS certification (or ability to obtain within 30 days of hire)
Ability to pass a DHS background check for designated positions
Knowledge of health center licensing and operations, particularly within FQHC environments
Ability to work both independently and collaboratively in team-based care settings
Demonstrated commitment to confidentiality, professionalism, and cultural sensitivity
Flexibility to work occasional evenings and weekends as needed
Professional and courteous demeanor with patients, colleagues, and supervisors
Must not appear on any State or Federal exclusion lists
Strong teamwork and communication skills
Physical Requirements
Frequent standing, walking, bending, and lifting
Frequent reaching, stooping, kneeling, and crouching
Ability to lift up to 50 pounds
Ability to assist with patient transfers and positioning
Adequate visual and auditory acuity for patient care and clinical documentation
