Pay Rate up to $38.45/hour.

Full Time Evening & Per Diem Days and Evenings.

Responsibilities

Read Daily Report at the beginning of shifts.

Supervises nursing care with particular attention to ill residents.

Takes vital signs on residents as needed.

Notes changes in condition in residents and notifies physician and medical POA.

Requisitions supplies and equipment. Orders medications according to facility routine.

Recommends changes in resident care when necessary and communicates this to the Director of Wellness.

Charts all pertinent information in resident's chart in a timely manner.

Calls physicians and pharmacies as needed to report discrepancies in orders.

Provides professional leadership to all nursing personnel.

Informs residents of any changes in their care.

Administers medication and treatments as ordered by the physician and charts appropriately when needed.

Assists nurses' aides and med techs as necessary.

Fills out incident reports when resident, visitor or employee is injured. Advises employees of facility policy regarding employee injury and proper medical attention.

Notifies the Director of Wellness of all incidents in a timely manner.

Fills out interagency referral forms.

Admits, discharges and transfers residents. Follows through assigning care for new residents.

Uses universal infection control precautions and ensures use by nursing personnel.

Provides our residents with daily care in accordance with our established policies and procedures and assures that the highest degree of quality care will always be maintained.

Qualifications

Experience in a memory care community is highly desirable.

Prior long‑term care experience is highly desirable.

CDP preferred.

Excellent communication and interpersonal skills are required.

Active RI Nursing License (LPN or RN) is required. Minimum of 3 years nursing experience.
