Base pay range:
$74,193.00/yr - $94,121.00/yr .
Additional compensation may apply based on shift assignment:
7% shift differential for evening shift and weekend days.
10% shift differential for night shift.
Duties:
Providing crisis intervention, service linkages, and discharge planning.
Developing treatment plans with individuals and treatment team; monitoring and evaluating progress; and modifying treatment plans as needed.
Determining discharge needs and appropriate community resources.
Establishing and maintaining relationships with community providers to ensure continuity of care.
Minimum Qualifications:
Licensed Master Social Worker (LMSW) registered with New York State.
Eligible under the Hiring for Emergency Limited Placement – Statewide (NY HELPS) program.
Or have one year of permanent competitive or 55-b/55-c service in a title eligible for transfer under Section 70.1 of the Civil Service Law if currently within State Service.

If you are appointed and you lose the ability to bill through Medicare and Medicaid, you may be terminated from your employment.
