About the Position We are looking for a qualified Physical Therapist (PT) in Minot, North Dakota (58701). The position is located at Trinity Health Town and Country – Minot, ND, with a schedule of 40 hrs/week (5x8 days).
Travel experience is required; recent travel experience strongly preferred. Cerner experience strongly preferred. Locals not accepted; must adhere to a 50‑mile radius rule.
Responsibilities
Provide rehabilitative services to home health and hospice patients.
Carry out skills and procedures that contribute to treatment, rehabilitation, and safety for home health patients and maintenance, education, and safety for hospice patients and families.
Plan coordinated therapy services for individuals and families under your care.
Appraise individual and family health needs and identify hazards—existing or potential.
Consult with and refer families to appropriate personnel within the agency or community services.
Execute treatment and preventative procedures under medical directions.
Work under the supervision of the supervisor/manager to evaluate, plan, implement, supervise, or document care of patients, with special attention to the geriatric population.
Qualifications
Licensed Physical Therapist in North Dakota.
At least 2 years of experience as a Physical Therapist (preferred).
Current driver’s license and vehicle available for travel to patients’ homes.
Experience with home health and hospice care (preferred).
