SWAN Outpatient Rehab – Physical Therapist Location: Phoenix, AZ (1190 E. Missouri Ave, Phoenix, AZ)

Position Type: PRN, no weekends or holidays

Hours: Flexible hours, no evenings, weekends, or holidays

Overview Provide 1:1 patient care in 60‑minute treatment sessions, focusing on functional rehabilitation for adults with neurological injuries. Collaborate closely with an interdisciplinary team of PT/OT/SLP/COTA/PTA to focus on patient‑centered outcomes. Provide personalized care in an innovative outpatient setting utilizing the latest technology and equipment including Bioness BITS, FES, biVABA, Assistive Technology, and more.

Responsibilities

Perform evaluations and develop treatment plans

Regularly re‑assess effectiveness of treatment plans; attend staff meetings and family conferences

Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources

Communicate patient’s needs and progress to the treatment team, physician, patient, and family members

Communicate with other disciplines to ensure collaboration, coordination of care, and enhance patient outcomes

Qualifications

Minimum of a bachelor’s degree in Physical Therapy from an accredited program

Current, unrestricted license as a Physical Therapist in the state of practice

Current basic life support or cardiopulmonary resuscitation certification in accordance with state regulations

A minimum of one year’s work experience as a physical therapist preferred

Demonstrated knowledge of rehabilitation techniques related to complex neurological injury

Effective verbal and written communication skills

Ability to lift 50 pounds; moving, lifting, or transferring of patients may involve lifting up to 100 pounds following safety procedures

Requires fine motor skills, visual acuity, and ability to walk/stand for extended periods

Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently
