Essential Job Functions Personnel
Training: Attend orientation and ongoing training; participate in monthly staff meetings.
Employee Scheduling: Work scheduled hours; arrive on time; stay on shift until replacement arrives; notify supervisor in advance when unable to work or as soon as possible after the start time; fill in for other shifts when required without causing unapproved overtime.
Teamwork: Work collaboratively, value others’ contributions, be courteous, communicate openly, listen, and show respect.
Workplace Safety: Comply with all safety policies, procedures, and practices; report all work‑related injuries to supervisor.
Services and Supports
Direct Support: Provide assistance in accordance with individual service or program plans; assist with socialization, behavioral development, personal care, housekeeping, recreational activities, transportation, community orientation, shopping, financial management, citizenship, and other activities of daily living.
Records: Accurately document progress and activity; review records and logs; maintain confidentiality; complete billing documentation timely.
Management of Individuals’ Assets: Assist with money management; ensure safety of individuals’ funds and property.
Relationships: Maintain healthy, professional relationships with individuals, families, guardians, and case managers; greet visitors; implement network customer service standards.
Reporting: Report alleged abuse or neglect; report medical, behavioral, and other incidents per network policy and external requirements.
Rights: Maintain confidentiality and respect the rights of those served, practice universal precautions, and assist individuals in exercising their rights.
Health Care
Appointments: Accompany individuals to medical appointments; relay orders and information to and from providers.
Medications: Accurately administer and document medications; report errors; maintain security of controlled substances.
General Health Care: Monitor health; document concerns; communicate with nurse or supervisor; call 911 in emergencies; follow health care directives.
Medical Supplies and Equipment: Follow procedures for equipment use; report malfunctions.
Dietary Planning: Prepare or support individuals in preparing meals per menus and individual choices; monitor mealtime and implement dining plans.
Maintenance
Vehicles: Transport individuals; drive safely; use safety equipment; report accidents immediately.
Housekeeping: Perform daily and seasonal housekeeping; maintain a neat, clean, safe environment; report hazards.
Maintenance and Repair: Monitor safety; make minor repairs; report equipment or facility repairs needed.
Safety: Check water temperature when assisting with bathing; participate in safety drills; protect individuals during emergencies.
Other
Perform other related duties and activities as required.
May occasionally use a personal cell phone for business; stipend provided.
Minimum Knowledge And Skills Required By The Job Education and Experience
High school diploma or equivalent; six months of human services experience preferred; must be 18 years of age.
Certificates, Licenses, and Registrations Required
Valid driver’s license in good standing; car registration and insurance if providing transportation.
All state‑required training and certification completed within mandated timeframes.
Blood‑Borne Pathogen Category
Job requires duties involving potential exposure to blood, body fluids, or tissues; such exposure is expected of employment.
Other Requirements
Travel required as needed.
Physical Requirements
Heavy work: Exert up to 100 pounds of force occasionally, up to 50 pounds frequently, and up to 20 pounds constantly to move objects.
