We seek a Case Manager to join our interdisciplinary clinical team at the South Mountain Health Center, working with individuals diagnosed with a Serious Mental Illness (SMI) or general mental health diagnosis under court‑ordered treatment.
Position details: Full‑time, 40 hours per week; located at 36th Street and Baseline. Competitive compensation ranging from $61,340 to $100,360 annually. New graduates welcome; no experience or bachelor’s degree in behavioral health required – we will train you. Bilingual Spanish is a plus and eligible for additional language differential pay.
Responsibilities
Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach those goals.
Completes assessments, crisis plans, and individualized service plans.
Works collaboratively with the clinical team to engage, educate, communicate, and coordinate care with the member, their family, behavioral health, medical and dental providers, community resources, and others to ensure all prescribed services are implemented.
Conducts outreach and follow‑up of services, including crisis interaction, missed appointments, and hospital discharges, to ensure adequate resources are in place.
Performs all case‑management functions associated with the caseload, including completing the assessment and service‑planning processes; identifies the need for further or specialty evaluations.
Pursues best‑practice outcomes for persons with mental illness, including continuing education, employment, independent housing, and community tenure.
Qualifications
Bilingual Spanish is a plus (additional language differential pay available)
Entry‑level position – no prior experience or bachelor's degree in behavioral health required – training provided
