Job Title: Behavioral Health Care Manager (Collaborative Care Model)
Location: Traverse City, MI (On-site at Primary Care Clinic)
Pay: $25.00 – $32.50 per hour (Part-Time to Full-Time)
About the Role Are you a licensed mental health professional looking to step away from the traditional "50-minute hour" and into a dynamic medical setting? Adaptive Counseling and Case Management is seeking a Behavioral Health Care Manager to join our team, embedded directly within a primary care practice.

True Collaboration: You aren't working in a silo. You will participate in weekly systematic case reviews with a Psychiatric Consultant (MD/DO).
Clinical Impact: You will use evidence-based brief interventions (Motivational Interviewing, Behavioral Activation) to help patients see real results.
Work-Life Balance: This is a primary care schedule. No nights, no weekends, and no on-call shifts.
Key Responsibilities
Manage a caseload of patients using a registry to track progress (e.g., monitoring PHQ-9 and GAD-7 scores).
Facilitate weekly caseload reviews with a Psychiatric Consultant to adjust treatment plans for patients not improving.
Provide brief, targeted behavioral interventions and counseling.
Coordinate directly with primary care providers to support medication management and adherence.
Connect patients with community resources when social barriers impact their health.
Requirements
Licensure: Active state license in Social Work (LMSW/LCSW), Counseling (LPC/LCPC), Nursing (RN), or Psychology.
Experience: Previous experience in case management, medical social work, or integrated health settings is highly preferred.
Skills: Strong organizational skills and comfort using Electronic Health Records (EHR) and registries.
Competitive Pay: $25.00–$32.50/hr.
