Western Tidewater CSB is seeking a Case Manager for our Early Intervention Program (age group, birth-3yrs) in Suffolk, VA.
Responsibilities and Duties for Early Intervention Case Manager
Consumer assessment
Service linkage
Coordination and development
implementation of a case management ISP
Assist and provide follow up instruction
Parent education
Monitor service delivery through contacts with consumers receiving services, family members, service providers, and periodic site/home visits
Manage consumer records according to all Licensure and Medicaid standards
Early Intervention Case Manager Qualifications
BA/BS degree from an accredited college or university with major course experience in Psychology, Social Work, or related field
One year of clinical experience in a Human Services field
Early Intervention Certification preferred
Valid driver’s license with acceptable DMV driving record at the time of employment and continuously, thereafter
Seniority level Mid-Senior level
