Nurse Practitioner – Skilled Nursing Facility
Schedule: Full-time, On-site, Monday–Friday Base Salary:
$100,000 – $150,000 per year Compensation and Benefits

Working collaboratively as a member of the medical staff, the APRN will play a critical role in managing the medical needs of residents within skilled nursing facilities. The APRN will build strong working relationships with administrators and staff throughout their designated building and serve as the primary point of contact for emergencies and urgent care visits. Ultra Health provides our APRNs with the training and tools to do more of what they love—working independently to see patients and treat complex medical conditions. Responsibilities
Provide primary, urgent, and preventative care for residents in assigned skilled nursing facility. May also provide care to assisted living facility residents Verify admission orders and conduct initial assessments of residents. Conduct physical exams, medication reviews, and depression screenings Collaborate with the facility medical director, attending physicians, and nursing staff to deliver the highest level of care Communicate with and counsel family members on treatment options when there are material changes in status Minimum Qualifications
Valid Nurse Practitioner License in applicable state DEA License Nurse Practitioner national certification Clinical background in adult, family, or geriatric specialties COVID-19 vaccination required unless qualified for a remote, religious, or medical exemption Preferred Qualifications
Experience in internal medicine, gerontology, or cardiology Excellent administrative, organizational, and communication skills Ability to work independently Detail-oriented, dependable, and reliable Nursing experience in LTC/SNF a plus Make a meaningful impact in the lives of seniors.
