Registered Nurse (RN) – 8‑hour day shift

Location: Elk Grove Village, IL
Schedule: 8‑hour day shift
Salary: $37.15 – $52.44 per hour

Responsibilities
Implement and monitor patient care plans.
Monitor, record and communicate patient condition as appropriate.
Perform direct participant care, including acute triage, blood draws, wound care, etc.
Participate in the development, implementation, and evaluation of the Interdisciplinary Team (IDT) plan of care.
Serve as a primary coordinator of all disciplines for well‑coordinated patient care.
Carry out physician and nursing orders.
Attend team meetings and care plan meetings.
Assess and coordinate patients' discharge planning needs with members of the healthcare team.
Participate, develop, and provide education to participants, caregivers, and associates.
Participate in after‑hours/weekend on‑call rotation, as needed.
May require travel to participant place of residence, long‑term care and assisted living facilities.
Requirements
BLS Provider obtained within 1 month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Driver's License credentialed from the Illinois Department of Motor Vehicles (IL‑DMV) obtained prior to hire date or job transfer date required.
Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required.
Education
Diploma from an accredited nursing school or professional licensure at the time of hire.
Additional Preferences
2 years of hospital or home health experience strongly preferred.
Sharp assessment skills, critical thinking, organizational skills, excellent communication skills and some computer knowledge.
Part‑time position allowing flexibility with an expectation of maintaining 20 credits per week.
Flexibility to work extra hours if agency needs. Agency hours are 8 a.m. – 5 p.m.
Opportunity to complete documentation at home after home visits.
Territory based on agency need; potential to case‑manage in the N. territory Hoffman Estates, Schaumburg, Streamwood and Elgin with expectations of weekend and holiday rotations.
