Description South Bend Orthopedics is seeking a PRN Physical Therapist for our Unity Hospital location. Unity is an elective orthopaedic surgical hospital.
Responsibilities and Duties
Assess patient condition, establish goals, treatments and care plan in conjunction with patient
Ensure appropriate treatment is performed in interdisciplinary fashion
Document findings, treatments, and other patient information for medical record
Educate patient on appropriate care and use of various therapies/exercises
Comply with appropriate clinic policies
Serve as information/education resource for patients, families, and staff.
Other duties as assigned.
Requirements
BS, MA, MS, or DPT in Physical Therapy
Must be licensed in the state of Indiana.

CPR Certification
