Base pay range $100,000.00/yr - $120,000.00/yr

Responsibilities

Serve as a clinical leader in a Skilled Nursing Facility (SNF) setting.

Collaborate with the facility’s care team to oversee short- and long-term residents.

Handle admissions, discharges, monthly stability checks, and evaluations for changes in condition.

Manage advanced care planning, comprehensive assessments, and prescribing treatments.
