Nurse Practitioner (NP) Integrated Kidney Care
Location: Chesapeake, VA / Norfolk, VA (Onsite across 3–8 dialysis centers)
Schedule: Full‑time, Monday–Friday | Daytime hours (8 AM – 5 PM)
Work Type: Onsite (no remote option)
As the founding NP in this market, you’ll shape a first‑of‑its‑kind initiative that integrates primary care, nephrology, and chronic disease management—bringing care to patients where they already receive dialysis. This is a clinical leadership role with significant autonomy and opportunities for innovation and future advancement.
Key Responsibilities
Deliver comprehensive primary care to patients within assigned DaVita dialysis centers (10–12 patient visits per day).
Manage chronic health conditions including diabetes, hypertension, COPD, heart failure, and other ESRD‑related complications.
Prescribe and titrate medications including insulin, antihypertensives, and statins according to evidence‑based guidelines.
Conduct annual wellness and preventive health evaluations.
Coordinate with nephrologists, dialysis care teams, integrated care leaders, and local PCPs to optimize care.
Develop clinical processes and workflows for a new integrated care model.
Participate in innovation feedback and collaborate directly with DaVita leadership on program design.
Qualifications Must‑Have Requirements
Active Nurse Practitioner (NP) License in Virginia with autonomous practice authority.
Minimum 5 years of clinical experience in Primary Care, Internal Medicine, or Gerontology.
Strong background in managing chronic conditions such as diabetes, hypertension, COPD, CHF, and kidney disease.
DEA license with proven prescribing experience for chronic disease management.
Local to Chesapeake/Norfolk, VA area (no relocation assistance).
Preferred Qualifications
Prior experience in dialysis, nephrology, or ESRD population management.
Experience launching or developing new clinical programs or service lines.
Comfortable working in a performance‑based, results‑driven model with clinical outcome incentives.
