As an Alternative Family Living (AFL) provider, you will open your home to provide an individual with intellectual and developmental disabilities a unique opportunity to live with a family unit. AFL living enables a person to live in a private home with a family and receive 24‑hour supports and habilitation.
Responsibilities
Provide a stable and caring home where the individual can live, grow, and integrate into the community working toward goals of daily living skills and independence.
Support the individual in daily living activities and assist with receiving services such as day programs or other services in addition to residential support.
Communicate with program staff and coordinate services as needed.
Qualifications
Experience as a Certified Nursing Assistant (CNA) or Direct Support Professional (DSP) preferred.
Ability to work with individuals possessing intellectual developmental disabilities and/or mental health challenges.
Strong commitment to the mission and values of Easterseals PORT Health.
