We’re looking for a dedicated Social Worker to provide supportive counseling, care coordination, and psychosocial assessments for individuals facing illness and disability.## ** Education & Licensure*** Master’s Degree in Social Work (MSW) required* Vermont **LMSW** required; **LICSW** preferred* At least 2 years of relevant experience (healthcare experience preferred)## ** Key Skills & Qualifications*** Strong communication, counseling, and rapport-building skills* Ability to assess psychosocial needs and explain medical information clearly* Experience with crisis intervention, community resources, and care management* Strong knowledge of local community resources* Knowledge of Medicare/Medicaid, insurance processes, and mandated reporting* Reliable transportation and valid driver’s license (travel required)* Ability to work collaboratively with patients, families, and interdisciplinary teams## ** What You’ll Do*** Complete comprehensive in-home psychosocial assessments* Support patients and families in coping with illness and disability* Coordinate care plans with medical team and community partners* Provide crisis intervention, short-term counseling, and education* Document assessments and care plans according to regulatory guidelines* Participate in team meetings, care conferences, and quality improvement activities## ** Working Conditions*** Work in patient homes, community settings, and varied environments* Occasional exposure to emotional situations or environmental stressors #J-18808-Ljbffr
