This position is eligible for a prorated $1,000 retention bonus.
Job Summary The Providence Center Residential Case Manager Brick Academy provides clinical case management services to facilitate program goals and objectives using a quality care approach. Identify client needs with a person-centered approach to address not only mental health symptoms but co-occurring disorders. Work with consumers to achieve the highest level of independence possible.
Duties and Responsibilities
Initiate treatment and advocate for consumer within network of care.
Assist in the development and implementation of socialization and recreational activities within the group home, in the community, and by attending Day Program groups and activities.
Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements, and abide by Center confidentiality policy.
Provide case management and support to consumers.
Make assessments and provide appropriate crisis interventions and risk management as needed to maintain a calm and safe environment.
Teach consumers adult daily living skills, assisting them in all aspects of their life in the group home and supervised apartments.
Transport consumers in own vehicle or TPC van (if available) to planned activities and appointments.
Maintain cooperative relationships with Center staff, consumers, community agencies, and the public.
Other duties as assigned.
Requirements
High School Diploma/GED required; Associate’s or Bachelor’s degree preferred.
If no higher education degree, must have 5 years of relevant behavioral health experience.
Experience in residential facilities is preferred.
Must have valid driver’s license, registration, current inspection and reliable, insured automobile with adequate liability coverage.
