Service Coordinator II (ACSW or AMFT)

to work in field, community, and clinic‑based settings serving TAY (Transition Age Youth) and adults living with mental illness, substance use challenges, and housing instability.

Here’s the truth: some days are heartwarming, some are chaotic, and some are… well, unpredictable. If you can roll with it, laugh through the challenges, and still get the work done, you’ll fit right in.

What You’ll Actually Be Doing Hunt down and engage clients where they are: shelters, streets, hospitals, jails, board & cares… you name it.

Provide clinical and case management support: therapy, crisis intervention, group sessions, and coordinated care planning.

Conduct comprehensive assessments, identify needs, and develop individualized plans.

Monitor clients for stability, intervene early when crises arise, and keep people safely housed.

Maintain meticulous documentation in Welligent/HMIS while meeting billing and program standards.

Participate in rotational 24/7 on‑call coverage — because emergencies don’t wait for a business card.

Collaborate with multidisciplinary teams, including DHS Intensive Case Managers, LVNs, PMs, and other Service Coordinators.

Who You Are

MSW or MFT, license‑eligible, and registered with CA BBS.

Experienced working with adults navigating mental illness, substance use, and housing challenges.

Comfortable working in unpredictable environments — sometimes messy, sometimes inspiring, sometimes both.

Skilled at building trust with people who may not give it easily.

Have reliable transportation and a valid CA driver’s license.
