Licensed Practical Nurse (LPN) Job Summary We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Wyomissing Health and Rehabilitation Center. As an LPN, you will play a vital role in providing high-quality patient care, working closely with our interdisciplinary team to ensure that our residents receive the best possible care.
Licensed Practical Nurse (LPN) Responsibilities
Provide direct patient care, including vital sign monitoring, medication administration, and wound care
Develop and implement individualized care plans in collaboration with the healthcare team
Conduct regular assessments and evaluations to identify changes in patient status
Communicate effectively with patients, families, and healthcare team members
Participate in quality improvement initiatives and maintain accurate records
Collaborate with physicians and other healthcare professionals to ensure comprehensive care
Licensed Practical Nurse (LPN) Requirements
Current LPN licensure in the state of Pennsylvania
Prior experience (new graduates welcome to apply)
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment
