You’ll help patients regain mobility and independence after surgery, making a real difference in their recovery.**Responsibilities*** Perform **home health physical therapy evaluations** and develop care plans* Provide therapeutic treatment and instruct patients/caregivers* Monitor progress and report changes to physicians* Participate in discharge planning and interdisciplinary case conferences* Supervise Physical Therapy Assistants per state regulations**Requirements*** Current **California Physical Therapy License*** 2+ years of physical therapy experience (**home health preferred**)* CPR certification* Valid driver’s license and insured vehicleWe’re part of the Cornerstone Group, with 75 agencies nationwide, but operate as a local team.
