SMI Case Manager - EMPACT
Location: City of Maricopa, Pinal County, AZ (25 miles southwest of Chandler)

Sign-on Bonus Up to $2000 (limited time incentive)
Position Overview As a Case Manager, you will work with our SMI Adult population, providing direct service crisis intervention, support, transportation, and education to families and children. You will conduct family needs assessments, make appropriate referrals, ensure continuity of care, and coordinate communications with other departments or collaborators. This is a full‑time position with flexible scheduling to meet client and family needs.
What You’ll Provide
Minimum of a bachelor’s degree in behavioral health related field or meeting BHT requirements.
Experience working with Severely Mentally Ill Adults and the adult system of care is required.
Ability to work flexible hours and meet individuals in their homes, community, or office.
Fingerprint clearance card or eligibility to obtain one.
Valid AZ driver’s license with no major infractions in the past three years.
Reliable transportation.
Bi‑lingual Spanish (preferred, commission per hour available).
Minimum age 21.
Additional Requirements and Responsibilities
Remain calm in crisis situations, work independently and cooperatively, recognize personal limitations, and relate positively to all persons regardless of age, race, creed, gender, or sexual orientation.
Maintain professional conduct consistent with ethical professional practice and agency policies.
Maintain a professional demeanor and positive working relationships with internal and external team members, stakeholders, and community partners.
When driving on agency time or business, possess a valid Arizona driver license, appropriate liability insurance, and approval from EMPACT’s liability insurance carrier. A 3‑year state driving record with no major infractions is required at time of hire.
Staff members who provide direct client services must be at least 21; support services staff must be at least 18. Minimum high school diploma or equivalent for all employees.
