Speech Therapist Home Health – PRN
Pinnacle Home Care, Florida’s largest Medicare‑certified home health provider, is hiring a PRN Speech Therapist (SLP) to deliver compassionate, patient‑centered care in the home.
Key Responsibilities
Evaluate patients’ speech, language, and hearing abilities, recommending strategies to enhance communication and hearing.
Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.
Collaborate with interdisciplinary team members on care plans and provide speech pathology training to team members as needed.
Communicate regularly with physicians, addressing any changes in patient condition.
Participate in periodic revision of care plans, updating the physician and care team on patient status and obtaining additional orders as required.
Educate patients and families/caregivers on speech therapy techniques and related challenges.
Confirm daily visit schedules to ensure seamless coordination with other staff.
Document all evaluation data, treatments, and patient progress promptly.
Supervise home health aides every 14 days when speech therapy is the sole discipline involved.
Qualifications
Must have a current SLP license (state‑specific).
Graduate of an accredited speech language pathology program.
Valid BLS certification.
Minimum two years of speech therapy experience (home health experience preferred).
Excellent communication, organizational, and time‑management skills.
Valid driver’s license and access to a reliable and insured vehicle.
Ability to visit homes that may not be wheelchair accessible.
Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
