Job Title: CenterWell Home Health LPN (Home Health Licensed Practical Nurse)

You’ll provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working closely with a dedicated team of physicians and clinicians, you’ll develop and manage care plans that support recovery and help patients get back to the life they love.
Responsibilities
Provide professional nursing services under the direction of a RN in compliance with state laws and CenterWell policies.
Deliver skilled nursing interventions for treatment, rehabilitation, and preventive care using a holistic approach.
Collaborate with RN Supervisor regarding treatment plan changes and communicate orders from physicians.
Maintain patient contact and coordinate care with physicians, clinical managers, and team.
Document care consistently and accurately in shift notes and visit records.
Qualifications
Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
Current nursing license in the practicing state.
Valid driver’s license, auto insurance, and reliable transportation.
Current CPR certification.
Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
Pay Range Base pay: $44,600 - $61,400 per year. Pay per visit/unit: $28.00 - $40.00.
