Responsibilities

Assess patients in their homes and develop and implement individualized care plans.

Monitor progress toward meaningful outcomes and adjust care as needed.

Administer medications and support wound care or infusion therapy as allowed by licensure and policy.

Manage chronic disease plans, perform vital signs, triage symptoms, and collaborate with physicians, therapists, and social workers to coordinate comprehensive care.

Provide thorough patient and family education on disease management, medication safety, fall prevention, and equipment use.

Ensure accurate documentation in the electronic health record and timely communication of changes in condition.
