Telos Health Systemsstaffs on a pay per encounter basis Psychotherapists, Licensed Mental Health Counselors, LMHC, Licensed Marriage and Family Therapist, LMFT, and Psychologists, PsyD or PhD in Skilled Nursing Facilities and Assisted Living Facilities to bring quality mental health services to the communities.
What You Will Do
Perform comprehensive diagnostic assessments of your patients’ needs and psychosocial status.
Develop a comprehensive, individualized treatment plan and goals for your patients.
Provide psychotherapy, group, and family therapy if applicable.
Collaborate with members of the facility’s interdisciplinary team, as appropriate, to develop treatment goals for the patient.
Provide appropriate documentation of all pertinent information obtained in interviews, consultation with other healthcare professionals, referrals, contacts, and supportive services provided to each patient.
Complete necessary documentation and reporting in accordance with departmental standards and Federal and State regulations.
Work in a role that does not include any case management or discharge planning; billing is handled by our billing team.
Schedule
Minimum 2 days a week
W2, Pay Per Encounter, Fee for Service role
Pay per encounter—just a 2-day minimum commitment per week out of M-F, prefer nonconsecutive days, and you choose the days!
Coverage Area Clearwater + Dade City + Brandon, FL
What You Will Need
Licensure / Certification – Full Licensure is required for our Psychotherapist roles
Education – Master's degree required
