The Case Manager is a key member of the Children’s Mental Health – Targeted Case Management (CMH‑TCM) team. The position is responsible for managing a caseload of 13‑17 children with severe emotional disturbances (SED). You will help recipients gain access to medical, social, education, vocation, financial and other necessary services as they relate to their mental health needs. Seventy‑five percent of a case manager’s available work hours will be spent on the direct service activities of client contact, collateral contact, coordination, documentation, IFCSP development and review, supervisor contact, transportation, and travel.
Essential Responsibilities
Complete written functional assessments and develop each child’s individual family community support plan (IFCSP) and update every 6 months
Create a crisis service plan
Meet face to face with the child and/or under certain circumstances a parent or guardian at least once every 30 days
Monitor the child’s progress toward achieving the outcomes specified in the child’s IFCSP, report progress toward these outcomes to the parent, child, and other members of the case management team every 90 days after the plan is developed, and revise the outcomes as appropriate based on the child’s progress toward the outcomes
Coordinate family community support services needed by the child and the child’s family with other services that the child and the child’s family are receiving
Arrange for a standardized assessment by a physician chosen by the child’s parent, legal representative, or the child of the side effects related to the administration of a child’s psychotropic medication
Be available to meet with the child’s parent or legal representative upon the request of the parent or representative
Meet weekly in person with youths who are out of home placement. Virtual meetings are possible if the youth is far outside of Ramsey County.
Actively participate in discharge planning for the child and, to the extent possible, coordinate the services necessary to assure a smooth transition to the child’s home or foster home, school, and community‑based services if the child is in a residential treatment facility, regional treatment center, correctional facility or other residential placement, or inpatient hospital for mental health services
Enter services into our Electronic Health Record and coordinate with Team Manager and Billing Manager to ensure timely billing for services.

Qualifications
Bachelor’s degree in social work or related field
At least one year of relevant experience providing case management services for children or youth
Commitment to providing excellent case management services for 15‑18 youth and their families
Demonstrated ability to communicate in a diverse cultural environment with strong interpersonal skills
Demonstrated cultural awareness.
Ability to pass criminal and sex offender background checks after a conditional job offer.
