As the Patient Care Manager – Hospice, you will assist the Executive Administrator, Executive Director, and Director of Clinical Operations with all patient care activities to ensure safe and efficient delivery of care that adheres to accepted nursing standards, regulatory requirements, medical practice guidelines, and payer criteria.

Primary Responsibilities:

Ensure compliance with hospice policies, procedures, and local, state, and federal regulatory requirements governing the practice of the assigned team.

Actively promote and direct the team regarding quality of care and safety of patients and staff.

Resolve problems thoroughly and investigate complaints.

Provide in‑services to staff to promote patient safety, familiarize staff with regulatory issues and agency policies, and address any identified educational needs.

Required Qualifications:

Current unrestricted RN licensure in the state of practice.

1+ year of clinical experience.

Current CPR certification.

Current driver’s license, vehicle insurance, and reliable vehicle or public transportation access.

Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Preferred Qualifications:

4+ years full‑time experience as an RN or equivalent.

Ability to work independently.

Good communication, writing, and organizational skills.

Pay is based on several factors including local labor markets, education, work experience, and certifications. In addition to salary, we offer a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution. The salary for this role ranges from $71,200 to $127,200 annually for full‑time employment. We comply with all applicable minimum wage laws.
