Clinical Pharmacist | Level 1 Trauma Center
Location:
On-site, Military Base, Fort Sam Houston, TX 78208
Position Type:
Permanent, Full-Time
Schedule:
Monday–Friday, 8:00 AM–5:00 PM CT
Compensation:
$57.69–$67.30/hour + $5K–$10K Sign-On Bonus
What You’ll Do:
Collaborate with multidisciplinary teams to optimize medication therapy and therapeutic outcomes.
Provide direct patient care, including medication assessments, therapy monitoring, and patient education.
Participate in emergency response, including Level 1 trauma activations, code blues, and massive transfusion protocols.
Review medication regimens for appropriate indications, dosing, interactions, and duplication of therapy.
Document pharmacy interventions, recommendations, and care plans in the medical record.
Support discharge planning and ensure continuity of care from a pharmacy perspective.
Develop, implement, and monitor clinical protocols.
Provide consultative expertise to healthcare providers to improve medication usage.
Monitor high-risk prescribing, including polypharmacy and high-dose regimens.
Promote cost-effective medication utilization within the clinic.
What You Bring:
Valid pharmacy license in Texas.
Bachelor's degree in pharmacy.
Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE).
Successful completion of an ASHP-accredited post‑graduate year one (PGY1) residency training program and ASHP-accredited post‑graduate year two (PGY2) or an ASHP‑accredited post‑graduate year one (PGY1) residency training program and three (3) years of clinical practice experience with BPS board certification.
Meets standards of an ASHP‑Qualified Residency Preceptor within one year of employment.
Current American Heart Association BLS and ACLS certifications.
Minimum of 1 year of clinical pharmacy experience within the past 3 years.
Valid driver’s license.
Ability to meet ASHP Residency Preceptor standards within one year of hire.
Physical & Work Requirements: Full‑time, permanent, on‑site role at a military base (travel may be required). Requires standing, walking, sitting, and bending. Occasional light lifting up to 20 pounds.
