Case Manager – Community Behavioral Health Clinic (ESP) – Athol, MA
Starting Salary: $38,000‑$44,000 per year.
Responsibilities
Identify clients needing case management, focusing on crisis stabilization and facilitating linkages to service supports including CSO, other mental‑health and SUD providers, medical providers, and wellness activities.
Engage crisis clients either at the time of the initial crisis encounter or within 24 hours through the client’s preferred method of contact.
Provide individualized support based on a person’s unique profile, including the individual’s support system, provider recommendations, and identified support needs.
Reach out to clients by telephone, in person, and through their support network.
Complete necessary documentation, releases, and required paperwork.
Assist in clarifying transition needs, gaps in services and supports.
Help locate housing, arrange transportation to appointments, and access supports for stabilization needs.
Encourage clients to use natural supports such as family and friends.
Practice a holistic and integrative approach to healthcare that includes physical, medical, and emotional wellness, covering social determinants of health.
Respond to client requests with flexibility and timeliness, creatively solving problems and maintaining a person‑first approach to care.
Collaborate with the client’s providers as needed within HIPAA guidelines.
Remain engaged with the client after the initial ESP encounter until the client is stable and engaged in appropriate community supports; consult with the clinical supervisor before ending support services.
Complete other assigned activities related to client care such as hospital searches, insurance verification, telephone triage, and other crisis duties.
Qualifications
Bachelor’s degree in a behavioral health discipline or a related field.
Experience working with adults experiencing mental health challenges and navigating the behavioral health system.
Valid driver’s license and reliable transportation.
