Case Manager – Substance Use Disorder Location:

Hampton, VA.

Schedule:

Monday–Friday 8:30 am–5:00 pm.

Salary:

$49,612 per year.

Responsibilities

Assess individual needs and develop tailored service plans for clients with substance use disorder.

Provide crisis intervention, basic needs assistance, and medication supervision.

Advocate for clients and coordinate services across disciplines.

Engage with individuals and families to deliver compassionate support.

Manage discharge planning and assist clients in achieving goals.

Qualifications

Bachelor’s degree in Human Services, Social Work, Psychology, Sociology, or Counseling.

Minimum one year of case‑management experience with clients diagnosed with substance use disorder or co‑occurring mental illness.

Valid Virginia driver’s license.

Knowledge, Skills & Abilities

Familiarity with community services and eligibility criteria for mental health, developmental disability, and substance‑use treatment programs.

Proficiency in assessment methods and service‑planning processes.

Knowledge of treatment and intervention approaches including behavior management, independent living skills, counseling, crisis intervention, discharge planning, and coordination.

Basic medication knowledge and understanding of applicable laws (HIPAA, 42 CFR Part 2).

Strong ability to identify and document individual needs; develop Individual Service Plans using assessments and interviews.

Capability to locate and coordinate community resources and natural supports.

Excellent teamwork and communication with community agencies.

Ability to work independently with general supervision.

Skill in building and maintaining supportive relationships with clients.

Background Checks The selected candidate must pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test, and employment reference checks.
