Job Summary & Qualifications The Case Manager provides case‑management services to families in VOA’s Housing programs. The role develops and monitors service plans to empower clients, increase self‑sufficiency, and reduce reoccurring homelessness. Responsibilities include connecting clients to medical, social, and educational resources, building landlord relationships, and communicating with property owners.
Qualifications:
Bachelor’s degree in social services preferred.
1–2 years of experience with crisis intervention and community resources.
Valid driver’s license and reliable transportation.
Flexibility to accommodate residential program schedules.
Strong communication skills.
Responsibilities
Knowledge of community resources for homeless families.
Develop specific housing opportunities for families in residence.
Develop cooperative arrangements with government agencies involved in providing housing.
Meet regularly with residents and assist them in applying for and following through with housing opportunities.
Maintain records and reports required by facility and oversight agencies.
Maintain individual client files including housing history and resource notices.
Quarterly reporting.
Performance quality improvement (PQI) duties as assigned.
Assist with facilitation of life skills as requested.
Other duties as assigned.
