CHC has been a critical part of Ohio’s efforts to treat and prevent substance use disorders since 1974. We are currently looking for a Case Manager for Touchstone, our Women's Residential Program. The Case Manager directs patients through substance abuse treatment through interactions with both CHC Addiction Services and the community. Responsibilities
Facilitating and coordinating program enrollment of women, children and infants Completing appropriate case recording requirements Providing patients with transportation Providing outreach services including public speaking/presentations and public awareness activities Minimum Requirements
• Bachelor’s Degree (Relevant professional experience may be considered in lieu of a degree). • CDCA or other license or credential that conforms to OhioMHAS regulations pertaining to counseling services. • Clean driving record. Benefits
