Salary Range:
$24.00 – $36.00 per hour
Travel:
Required to support multiple locations within a 150‑mile radius of permanent address. Mileage reimbursement provided for required travel.
Premium Pay Rates:
$5.00 per hour premium on all hours worked; $1.25 per hour weekend shift differential; $1.25 per hour evening/night shift differential (6:00 PM – 6:00 AM).
Experience Requirements:
Minimum 3 months in Long Term Care (6 months preferred).
Scheduling Requirements:
Minimum 30 hours per month; one weekend shift per month.
Job Summary The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long‑term care under the supervision of a registered nurse, advanced practice provider, or physician. The LPN demonstrates knowledge of nursing principles and the ability to perform tests, recognize problems, recommend solutions, and respond effectively to emergencies. Responsibilities include ensuring the health, comfort, and safety of residents; contributing to assessments; administering medications and treatments; providing technical support; documenting interactions; establishing effective relationships with residents and the interdisciplinary care team; counseling residents, families, and advocates; and performing other related duties as assigned.
Qualifications Graduate from an accredited practical nursing program as a Licensed Practical Nurse. Associate degree in practical nursing preferred.
Minimum of 18 years of age.
Attended a pre‑licensure nursing program with supervised clinical experience across the life span or 400 hours of experience as an LPN at another facility.
Currently holds an unencumbered Licensed Practical Nurse (LPN) license with a state nursing board and/or possesses multi‑state licensure privileges.
Obtains and maintains required department‑specific competencies and certifications, including required annual continuing education units (CEU) and competency training.
