Licensed Practical Nurse Position Summary Part time, PRN
Responsibilities & Accountabilities
Coordinate the day‑to‑day care of residents in accordance with the service plan. This includes ensuring sufficient staffing, coordinating staff schedules, and supervising RMAs and direct‑care staff.
Be knowledgeable of each resident’s individualized care plan and provide support accordingly. Contribute to the care‑planning process by providing the charge nurse or other care‑planning staff with specific information and observations of the residents’ needs and preferences.
Complete any state‑required assessment and service‑plan training.
Coordinate services with hospice and home health to ensure that they provide services as ordered by the primary care providers and outlined in the service plan. This includes reviewing outside‑provider communication and participating in wound monitoring/management per Viva Senior Living policies.
Coordinate services with durable medical equipment companies to ensure residents receive the medical equipment needed as directed or ordered by their medical provider.
Coordinate medication management with pharmacies, including managing medication orders, ensuring availability of medications, medication destruction, medication cart management, and routine auditing of medication services.
Administer and document medication and treatments per the physician’s order and accurately record all care provided to the residents.
Contribute to continuity of care and effective communication by reporting care provided and changes in the resident’s condition to the responsible party, primary‑care provider/physician, and Resident Care Director.
Review end‑of‑shift reports and Med‑Tech logs and respond accordingly.
Respond to resident emergencies to ensure an appropriate response and follow‑through.
Document incident reports as needed in accordance with Viva Senior Living policies.
Conduct community audits and training as needed to ensure compliance with state regulations and company policies.
May participate in routine activities of residents’ care, including daily living and basic nursing procedures.
Respond promptly and courteously to inquiries from residents, visitors, and other personnel within the required time frames and established policy.
Communicate and interact effectively and tactfully with residents, visitors, families, and peers.
Arrive to work at the scheduled time.
Perform other related duties as required.
Ensure that residents and families receive the highest quality of service in a caring and compassionate atmosphere that recognizes the individuals’ needs and rights.
Report any occupational exposures to blood, body fluids, infectious materials, or hazardous chemicals in accordance with facility policy.
Report work‑related injuries and illnesses immediately to your supervisor.
Complete all assigned training and skills competency as a condition of employment.
Participate in all life‑safety and emergency drills and trainings.
Fulfill responsibilities as assigned during implementation or activation of the facility’s emergency plan.
Assist with the evacuation of residents in an emergency situation.
Maintain confidentiality of protected health information, including verbal, written, and electronic communications.
Report non‑compliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel.
Protect residents from abuse and cooperate with all investigations.
Report allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to the supervisor and/or administrator.
Report any retaliation or discrimination to the Human Resource Director and the Compliance Officer.
Report any suspicion of a crime to law enforcement and the state agency.
Perform other duties as requested.
Requirements
Must possess a valid LPN license in good standing in accordance with the laws of the state, issued by an approved TN school of nursing.
Comply with standards of business conduct in accordance with federal, state, and local health and regulatory standards and guidelines, as applicable.
Must possess, at a minimum, a high school diploma or a GED.
Must be able to work well with peers in a congenial work environment, and above all, maintain the utmost compassion and dignity toward the residents and adhere to the Resident Bill of Rights.
In exigent circumstances, the employee may be called back during emergency conditions (e.g., severe weather, evacuation, temporary staffing shortages, etc.).
Must adhere to Viva Senior Living’s attendance policies and follow the terms and conditions of employment as set forth in the Employee Handbook.
Must be able to read, write, speak, and understand English.
Must maintain the highest standards in caring and servicing the needs of residents and their families and loved ones.
Must adhere to all facility policies and procedures.
Must be in good standing with the state Board of Nursing and hold an active, unrestricted LPN license.
Must have a clean work‑history record with no history of disciplinary action.
Annual Health Requirements
Provide annual verification of a negative TB skin test.
Physical & Sensory Requirements
Must be able to move intermittently throughout the workday.
Must be able to cope with the mental and emotional stress of the position.
Must be able to see and hear, or use prosthetics that enable the senses to function adequately to ensure that the position’s requirements are fully met.
Must meet the general health requirements set forth by the policy of the facility, including a medical and physical examination.
Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and, at times, agitated residents.
Must be able to push, pull, move, and/or lift a minimum of 25 pounds to at least 5 feet and be able to carry such weight over a minimum distance of 50 feet.
May be required to assist in the evacuation of residents during emergency situations.
