PAY AND BENEFITS Hourly Pay Range:

$25.619109- $32.359609

Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

Bilingual required positions receive an additional

5% add-to-pay.

JOB DETAILS AND QUALIFICATIONS The

Children, Families and Community Connection Division

is looking to fill a bilingual Spanish/English position as a Case Manager Aide. This is a new limited term position supporting a new program, Housing for Success (H4S). The duration of employment will be three (3) years.

The Case Manager Aide plays a vital support role within the Housing for Success (H4S) Program, assisting with participant enrollment, data collection, scheduling, and coordination with community partners. This position helps ensure participants have access to needed resources and services while providing administrative and technological support to the Workforce Specialist and Mental Health Navigator.

Serving as a client advocate, the Case Manager Aide builds supportive, trusting relationships with program participants and contributes to positive outcomes through consistent follow-up, attention to detail, and the maintenance of accurate and confidential records. This role is ideal for someone who is compassionate, organized, and committed to supporting individuals on their path to stability and success.

Required Minimum Qualifications/ Transferrable Skills:

A minimum of one (1) year of related experience that would provide the required knowledge and skills to perform the responsibilities of this position.

Experience to include providing client support, case management assistance, intake, eligibility screening, and/or administrative support in a human services, housing, workforce development, or social services setting.

Bilingual Spanish/English.

Ability to provide administrative and basic technology support to clients, including scheduling appointments, collecting information, maintaining accurate and confidential electronic case records, and assisting clients with computer, email, document management, and smartphone use.

Other Required Knowledge, Skills & Abilities:

Ability to schedule client appointments, collect information and maintain electronic client case records in a complete, accurate, timely and confidential manner.

Ability to support clients with technology needs including basic computer set up, email, creating/saving/attaching documents and setting up smart phone.

Ability to assist clients to fill out a variety of forms online or on paper.

Knowledge of case management principles and client support services.

Ability to interview clients and assess needs related to employment, training, and supportive services.

Ability to maintain accurate, organized, and confidential records.

Strong interpersonal and communication skills, both written and verbal.

Ability to work effectively with diverse populations and community partners.

Proficiency with data entry, computerized case management systems, and basic reporting tools.

Ability to verify information and exercise sound judgment.

Strong organizational skills and attention to detail.

Ability to manage multiple tasks and meet deadlines under general supervision.

Familiarity with community resources and service provider networks.

Preferred Special Qualifications/ Transferrable Skills:

Experience working in human services, housing, workforce development, or social services programs.

Experience working directly with clients in office and community-based settings.

Experience hosting online meetings using Microsoft Teams and/or Zoom

Experience using principles of trauma informed care and deescalation techniques.

Familiarity with community resources and service provider networks.

TYPICAL TASKS

Screen client cases to determine appropriate assistance and service needs.

Conduct client interviews in office settings, community locations, and by phone.

Assess client employment, training, personal, and community support needs.

Provide information, referrals, and technical assistance to clients and staff.

Prepare, update, and maintain accurate and confidential client case records.

Monitor client case files to ensure delivery of services established by professional staff.

Verify accuracy of information provided by clients, service providers, and related parties. Enter client assessments and required documentation into computerized systems.

Generate reports related to client assessments and program data.

Serve as a liaison between clients and service providers to resolve issues related to services, paperwork, or records.

Research community resources and potential service providers.

Collect program data and assist with reports, evaluations, and special studies.

Participate in in-service training and provide casework consultation as needed.

Manage a small caseload under general supervision.

Assist with employment plan development and write case file narratives.

Respond to client and provider inquiries and help resolve problems.

WORKING CONDITIONS Duties may include contact with individuals from varied socioeconomic groups or individuals who are hostile and/or emotionally distressed and in need of Clackamas County services.

WORK SCHEDULE This position is included in the County's alternate workweek program,

working 37.5 hours during a standard workweek of Monday through Thursday (Fridays off) . Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.

This

position is designated as on-site/in-person.

It is expected that the selected candidate will perform duties at the specified on-site work location.
