Position Summary The Licensed Practical Nurse (LPN) or Registered Nurse (RN) reports directly to the Director of Nursing and is responsible for delivering, coordinating, and evaluating resident care in alignment with physician orders, individualized service plans, and regulatory standards. This role combines hands‑on clinical responsibilities—including medication administration, assessments, documentation, and direct resident support—with leadership functions such as guiding care staff, promoting safety initiatives, and ensuring effective communication with residents, families, physicians, and community leadership.

Essential Functions

Provide services in accordance with the nurse practice act and state/federal nursing center regulations.

Work with the Director of Nursing to provide oversight to the care staff regarding resident needs.

Review and revise assessments and care plans at admission, periodically per regulation, and at change in resident physical or psychosocial condition.

Review and transcribe physician's orders and provide medical treatments as ordered.

Initiate a resident education plan per individualized needs and physician or community policy.

Observe residents and communicate condition changes to physicians, health care proxies, and supervisors; provide timely interventions as instructed.

Administer and document medications in accordance with the five rights.

Supervise and monitor medications for residents who self‑administer when applicable.

Document all pertinent observations, red flags, incidents, interventions, communications, and clinical procedures throughout the shift.

Assist and monitor residents with meeting nutritional/hydration needs, documenting on the nursing record.

Assist residents with activities of daily living as needed (bathing/showering, oral care, toileting, dressing).

Assist residents in scheduling medical, dental, and laboratory appointments.

Utilize proactive interventions and monitor safety during rooms and rounds.

Answer resident call light promptly.

Assist residents with transfers and ambulation using proper body mechanics.

Take temperature, pulse, blood pressure, and vital signs to detect deviations from normal.

Maintain a restraint‑free environment at all times.

Support fall prevention programs for all residents in need.

Respond to life‑saving situations based on nursing standards, policies, procedures, and protocol.

Provide first aid, call emergency personnel, and/or call family members as requested.

Review resident service plans for assisted living and memory care.

Encourage residents to participate in scheduled facility activities and provide assistance as needed.

Assist in post‑mortem care as instructed.

Participate in oversight and management of medication delivery system, timely order entry, receipt, tracking, MAR documentation, narcotic count, and medication compliance.

Notify pharmacy and supervisor of issues and seek resolution promptly.

Promote physical, personal, and emotional well‑being of each resident, striving to support maximum independence when possible.

Understand residents' Advance Directives and follow their wishes.

Ensure effective communication with residents’ families about residents’ care and well‑being.

Work well with others, take direction from management, and resolve conflicts as needed.

Perform other duties as assigned.

Qualifications

Graduation from an accredited nursing school and a current state license for LPN or RN.

1+ years of nursing experience preferred; new graduates welcome.

CPR or BLS certification preferred.

Proficiency in Microsoft Office, web‑based platforms, electronic health record systems, and HR‑related systems.

Working Conditions

May need to walk or stand for extended periods and move throughout the community.

May occasionally stoop, kneel, crouch, or crawl, and climb or balance.

Requires effective communication and written information interpretation.

May need to lift up to 50 pounds independently, and up to 200 pounds with assistance.

Required to follow infection prevention and control protocols, including PPE use.

Actively participate in staff activities fostering teamwork and morale.

Must stay current on all training and education initiatives, pursuing self‑improvement.
