Case Management Coordinator | Iredell Davis Behavioral Health | FT
Location: Statesville, NC.
Description The Case Management Coordinator is responsible for establishing, coordinating, and maintaining the process of inpatient admissions, pre‑certifications, addressing insurance denials, collaborating with LLM, overseeing discharge planning, and ensuring patients have a safe discharge plan. The Coordinator collaborates with Intake Nurses, Utilization Review, Recreational Therapists, Social Workers, Patients/Family, Physicians, community resources, and payers to ensure the patient’s progress and level of care is appropriately determined. They have well‑developed knowledge and skills in patient status determination, assessment, and care management within the inpatient setting. Responsibilities include patient/family assessment and management, resource management, identifying patients appropriate for inpatient admission, discharge planning with referrals to all levels of care, and other related duties specific to this defined patient population. The Coordinator utilizes knowledge of pathophysiology, pharmacology, and clinical care processes to participate with clinical staff and physicians in developing clinical practice guidelines and physician order sets for improving quality of care, changing practice, and reducing costs, with full access to patient health information.
Requirements
Current Registered Nurse licensure in North Carolina
ASN or BSN degree from an accredited institution
3‑5 years experience in a clinical setting
Experience in case management preferred
Project management skills
Knowledgeable about Behavioral Health reporting requirements—including NHSN, in‑patient quality reporting for CMS and the Joint Commission
Experience with payer contracts, LLMs, and insurance denial processes
Knowledgeable about current healthcare regulatory standards
Excellent verbal, written, communication, and organizational skills
Knowledge of computer applications—Word, Excel, etc.
Essential Physical Requirements
Must possess full range of body motion to pass a basic FIT test, including bending, stooping, standing, and sitting for extended periods of time
Must be able to lift and carry up to 30 pounds
