Key Responsibilities
Evaluate and diagnose communication, cognitive, and swallowing disorders in adult and geriatric populations.
Develop and implement individualized treatment plans to maximize resident outcomes.
Collaborate with a multidisciplinary team, including nursing, physical therapy, occupational therapy, and social work.
Document all services provided in accordance with facility and regulatory standards.
Educate residents, family members, and staff on communication strategies and safe swallowing techniques.
Participate in care planning meetings and contribute to discharge planning when appropriate.
Qualifications
Master’s degree in Speech-Language Pathology from an accredited program.
Current SLP license in the state of Ohio
Certificate of Clinical Competence (CCC-SLP) from ASHA preferred
Experience in a Skilled Nursing Facility (SNF) or long-term care setting preferred, but not required.
Strong communication, organizational, and clinical reasoning skills.
Compassionate, patient-centered, and team-oriented.
