Pay Range: $28.69 - $37.38 hourly.

Overview We seek a visionary and dynamic leader to join our Integrated Clinical Services team as a Licensed Practical Nurse (LPN). The role focuses on providing focused community and clinic health services, prevention of disease, health promotion, and chronic disease management. The Panel Manager tracks the health of the provider panel, manages team communication, oversees electronic health record InBasket, and supports client visits.
Responsibilities Panel Management
Utilizes electronic reports and reviews provider schedules to proactively assess prevention needs and communicate via EHR.
Performs pre-visit activities (scrubbing, huddling) as needed.
Monitors chronic disease and prevention reports, initiates client follow-up or referrals.
Forecasts immunizations.
Collaborates with Team Clerical Assistant to track referrals.
Leads team and directs staff within scope to support smooth clinic flow.
Team / Client Communication
Manages team cell phone, desk phone, lab results, and messages.
Manages provider and team electronic and paper InBaskets, schedules appointments.
Follows up on specialty referrals, prescriptions, cancellations.
Participates in patient reminder calls and follow‑up to no‑shows.
Documents clearly using SOAP method and EPIC "dot‑phrases" as appropriate.
Communicates professionally with all staff.
Provider / Team Support
Supports back‑office clinic flow, rooming patients, ordering labs, providing immunizations and medications.
Assists clinicians with procedures and provides after‑visit instructions.
Obtains vital signs and screenings, collects assessment data on chronic conditions.
Prepares patients for exams, sets up equipment, collects specimens.
Administers medical treatments (wound care, irrigations, suture removal, catheterizations).
Performs diagnostic tests: labs, phlebotomy, EKG, etc.
Completes Nurse standing orders as assigned and provides patient education.
Assists clients with forms and follows established plan of care.
Maintains clinical skills, reviews cases with providers, and seeks expert advice.
Completes required competencies and documentation.
Resource Management & Quality Assurance
Participates in equipment and supply checks.
Troubleshoots equipment issues and reports repairs.
Provides feedback for communication flow.
Provides backup support to other team members and completes Quality Control activities.
Actively participates in process improvement projects.
May perform Vaccine Lead duties if assigned.
Qualifications Minimum Qualifications
Associate's degree or Bachelor's in Nursing from an accredited program.
Valid Oregon Licensed Practical Nurse license by hire.
Minimum one year of nursing experience.
BLS/CPR certification.
Preferred Qualifications
Experience providing services to diverse and vulnerable communities.
Demonstrated primary care experience.
Experience with Medical Home or FQHC.
Additional transferable skills and education relevant to the role.
