Position Summary This position is responsible for the evaluation, planning, administration, and supervision of Physical Therapy treatment in accordance with physician referral, state and national standards, regulatory agencies, and established departmental policies and procedures. The role requires clinical competence and knowledge to provide treatment appropriate to patients of all ages.
Core Functions
Evaluate patient's functional abilities and limitations; determine if intervention is needed; establish a plan of care and treatment goals; complete comprehensive evaluation within the established time frame; identify and prioritize key limiting factors; collaborate with patient and family/caregiver when setting goals; initiate discharge planning; recommend additions or modifications to referring orders.
Implement individualized treatment plan considering the patient's physical, social, spiritual, cultural, educational, and age-specific needs; provide skilled treatment safely; assess ongoing effectiveness of treatment and modify plan of care as indicated; involve patient and family/caregiver in treatment and decision making; provide ongoing education to physician, nurse, patient, family/caregiver, and interdisciplinary team.
Document according to professional practice guidelines; document legibly; complete documentation within established department time frames; record evidence-based and individualized treatment techniques/plan; ensure documentation reflects skilled intervention and comprehensive assessment of patient progress.
Communicate pertinent patient information promptly to ensure best possible follow-through of care; discuss evaluation/treatment results and patient progress with physicians, nurses, patients, family/caregivers, and the interdisciplinary team; contact physicians for routine or problem-solving to ensure optimal outcomes; defend treatment rationale with standard practices and current literature.
Show adaptability in assignments related to caseload, scheduling, and staffing shifts; take initiative to assist in other patient care areas; accept responsibility for all patient care needs.
Demonstrate appropriate supervision and delegation of personnel to ensure safe delivery of quality, cost-effective patient care, including PTA, ATC, Therapy Technician, and student; participate in training and orientation of new personnel; assure competency prior to delegation.
Participate in department-required meetings and process improvement; meet productivity standards set by the department.
Interact with all levels of staff in various departments, physicians, patients, families, and external contacts, including physician offices, to report, request, or clarify information.
Training Note: Training for this position will be conducted during weekdays.
Minimum Qualifications
Graduate from an APTA accredited school of Physical Therapy or equivalent.
Current Physical Therapy license in the State of practice.
BLS certification required.
Demonstrate clinical competency, experience, training, and education through interview and job-related experiential questions.
If employed at Banner Home Care/Hospice or Boswell Skilled Nursing Facility, possess an Arizona Fingerprint Clearance Card at time of hire and maintain it during employment.
Proficiency with computer software applications commonly used in an automated patient care environment (e.g., patient care equipment, evaluation tools, therapeutic modalities, copier, multi-line telephone, fax, computer, pager).
Preferred Qualifications Additional related education and/or experience preferred.
