Schedule Rotating Days/Evenings with Every Other Weekend Required. Weekdays: 6am-1pm, 12:30pm-9:00pm, 12:00pm-8:30pm. Weekends rotating shifts 7am-3:30pm or 3pm-11:30pm.
Summary Home care opportunity where LPNs work to the top of their license by administering care to ElderONE participants in a variety of settings. The community LPN will participate in a team approach to meet the needs of the participants in their homes, or in transitional housing. This may include home visits for dressing changes and medication management and then being assigned to a transitional housing location. These are locations where there are 3-8 participants residing in one place and are in this setting to avoid hospitalization and skilled nursing facility visits, keeping them in the community until they are ready to return to their own residence. The LPN plays a vital role in the participant’s success.
Responsibilities
Preferred qualifications: home care is preferred but we can work with the right person to gain skills and experience in home care. A desire and passion to work with elderly participants in a variety of settings (outside of Skilled Nursing Facilities).
The community LPN will participate in a team approach to meet the needs of the participants in their homes, or in transitional housing. This may include home visits for dressing changes and medication management and then being assigned to a transitional housing location.
Minimum Qualifications
Graduate from an accredited School of Practical Nursing or equivalent required.
One year experience working with frail elderly population required.
NYS Driver's License.

Licenses / Certifications BLS - Basic Life Support - American Heart Association (AHA) LPN - Licensed Practical Nurse - New York State Education Department (NYSED)
Physical Requirements M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
Pay Range $27.75 - $33.75 per hour.
