Hourly Wage Estimate: $40.15 - $58.58 / hour
Job Summary Responsible and accountable for coordination of services through an interdisciplinary process that provides a clinical, financial, and psychosocial approach through the continuum of care. Performs concurrent clinical reviews to determine appropriateness of admission and continued need for care delivery in the acute care setting. Conducts telephonic clinical reviews with claims insurance personnel to facilitate reimbursement for patient care services. Assesses patients for discharge planning needs and coordinates referrals to the interdisciplinary team. Supports collaboration and communication among patients, families, and the healthcare team regarding ongoing care needs and facilitates necessary services. Identifies quality and risk issues and reports them to the appropriate individual or department.
Responsibilities
Guide patients and families through program orientation, explaining rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights.
Coordinate education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences.
Develop and implement individualized treatment plans that reflect each patient’s strengths, needs, and personal recovery goals.
Complete psychosocial assessments and discharge planning evaluations, clearly documenting findings and communicating needs across the care team.
Collaborate closely with Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care.
Promote HCA Healthcare’s values of compassion, respect, and excellence through every patient and family interaction.
Qualifications
Registered Nurse (RN) with valid license.
Basic Cardiac Life Support (BCLS) certification.
