Job Description Summary The Registered Nurse plans, organizes, and directs home‑care services, focusing on community health education and patient/family-centered care. The professional nurse utilizes community resources to meet the needs of individuals and families in their homes and communities.
Essential Job Functions / Responsibilities Patient Care
Completes an initial assessment of patient and family to determine home‑care needs, including a full physical assessment and history.
Regularly re‑evaluates patient nursing needs.
Initiates plan of care and makes necessary revisions as patient status changes.
Uses health assessment data to determine nursing diagnosis.
Develops a care plan with goals based on nursing diagnosis, incorporating therapeutic, preventive, and rehabilitative actions.
Initiates preventive and rehabilitative procedures; administers medications and treatments as prescribed.
Counsels patient and family in meeting nursing and related needs.
Provides health‑care instructions appropriate to assessment and plan.
Identifies discharge‑planning needs and implements them before discharge.
Communication
Prepares clinical notes daily and updates primary physician at least every sixty days.
Communicates patient needs and changes in condition to physicians, obtaining orders as required.
Coordinates care plan with community health personnel.
Additional Duties
Participates in on‑call duties.
Ensures arrangements for equipment and other supplies are available.
Instructs, supervises, and evaluates home‑health aide care every fourteen days.
Position Qualifications
Registered nurse licensed through the State Board of Nursing.
Current CPR certification and a valid driver’s license.
At least one year of general nursing experience (home health, clinical, surgical or critical care).
Self‑directed, works with little supervision; good organizational skills.
Excellent observation, verbal and written communication, problem‑solving, basic math, and nursing skills per competency checklist.
Physical ability: prolonged walking/standing, lifting, positioning, transferring patients and supplies, reaching, stooping, bending, kneeling, crouching; adequate visual acuity and hearing.
Licensed driver with an insured automobile in good working order.
Flexible and cooperative in fulfilling role obligations.
