CHD is seeking a passionate and dedicated

Psychiatric Coordination Clinician

to support assessment, treatment planning, and evaluation for clients receiving psychiatric medication management. You will work with adults, children, and families who are seen by a psychiatry provider and do not require frequent outpatient therapy. You will support psychiatric providers in creating and maintaining treatment plans and providing up to date assessments for clients.

About the Role CHD is seeking a passionate and dedicated clinician to support assessment, treatment planning, and evaluation for clients receiving psychiatric medication management. You will work with adults, children, and families who are seen by a psychiatry provider and do not require frequent outpatient therapy. You will support psychiatric providers in creating and maintaining treatment plans and providing up to date assessments for clients. This position can be stationed primarily within our Greenfield, Orange, or Easthampton offices with a willingness to travel between them at least 3 days per month.

Key Responsibilities

Provide annual comprehensive assessments to clients receiving psychiatric services with an emphasis on evaluating level of care needs.

Engage stable clients in routine check-in appointments.

Establish and update treatment plans on a collaborative basis with clients and other providers.

Discuss and submit referrals to other supportive services.

Ensure confidentiality and adhere to all privacy regulations.

Address client safety needs and follow mandated reporting procedures when necessary.

Build and maintain a strong collaborative relationship with both internal and external treatment teams as appropriate.

Maintain accurate and timely clinical documentation.

Adapt to updates in policies and procedures as required.

Engage professionally with clients, colleagues, and supervisors.

Requirements

Master’s Degree in Social Work, Mental Health Counseling, or other closely related field.

Massachusetts Independent Behavioral Health License (i.e. LICSW, LMHC, LMFT, LADC1) OR actively working toward licensure.

1+ years in an outpatient behavioral health setting.

CANS Certification or willingness to become certified prior to hire.

Strong communication, organizational, collaboration and documentation skills.

Preferred Qualifications / Abilities

Experience with children, adolescents, and families.

Familiarity with Electronic Health Record (HER) systems.

Multilingual abilities.

Experience with Substance Use Disorders.

Locations Franklin & Hampshire Counties (Orange, Greenfield, and Easthampton Clinics) – primarily stationed within one clinic with willingness to travel between locations occasionally.

Hours 40 hours, Monday-Thursday or Monday-Friday.
