About The Role Longevity Health is looking for a Nurse Practitioner or Physician Assistant with a passion for working with seniors in long‑term care settings. In this role you will be responsible for a panel of enrolled members receiving nursing home care and will be supported by a dedicated clinical leadership team that provides education and guidance on best practices for effectively caring for our members. You will conduct comprehensive assessments, develop individualized care plans, actively participate in nursing facility care team meetings, maintain frequent communication with members and families, and ensure the care team stays aligned with members’ goals and needs.
Key Tasks and Responsibilities
Provides collaborative primary care and services to a panel of Longevity Health members, treating in the most appropriate setting based on the member’s goals of care.
Executes and delivers contracted plan’s Plan Model of Care activities, develops and implements advanced care planning, and embraces the treating‑in‑place model.
Facilitates and administers health plan clinical operations functions—prior authorization, Part A and B services, medication reviews, HEDIS and STARs quality gap closure, promoting membership growth.
Serves as a trusted advisor to members, loved ones, and nursing home staff; educates care teams on member needs and early identification of changes in condition and notifies APP and telehealth partners for member changes.
Delivers effective and frequent communication and coordination with members’ PCPs, loved ones and care team members on members’ care needs, goals of care, and treatment plans or changes.
Supports overall market goals and targets and balances member caseload demands with other team member support needs.
Promotes Longevity Health with members, loved ones and nursing facilities and other stakeholders.
Can function autonomously and is accountable for practice.
Establishes meaningful relationships and can function in the nursing home environment.
Is member‑centric and customer‑service oriented, establishing meaningful relationships that ensure effective and high‑quality care for Longevity Health members.
Performs other related duties as assigned.
Supervisory Responsibilities
There are no supervisory responsibilities for this position.
Credentials & Coverage
State‑unrestricted licensed Advanced Practice Nurse, National Board certification (AANP, ANCC or AACN) as an Adult, Family, Geriatric or Primary Care nurse practitioner; or state‑unrestricted licensed Physician’s Assistant NCCPA board certified with nursing home/geriatric or hospice/palliative experience.
Active DEA license registered in the state of practice is required.
Must have an active national provider number (NPI) and an active Medicare provider number.
Must be eligible to participate in the Medicare program as a provider and registered.
Valid state driver’s license with a good driving record and proof of automobile insurance required.
Auto insurance coverage as required by your state of residence. New employees must provide proof of coverage prior to hire and on an ongoing basis and must be willing to drive up to one hour to and from work daily.
Must be present in SNF communities/buildings during business hours daily.
Education And Training
Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program, or Master’s Degree from a nationally accredited program.
Knowledge And Experience
Minimum of 6 months experience as a Nurse Practitioner/Physician’s Assistant and some experience with geriatric, hospice or palliative care populations prior to becoming an NP/PA, or a licensed and board‑certified new graduate NP/PA supervised by an experienced NP/PA in the LHP New Graduate Fellowship program for 6 months and successfully completing all competencies for independent practice as an LHP APP.
Experience working in the nursing home environment is strongly desired.
Computer/technology proficiency required.
Excellent people skills—ability to engage and communicate effectively with physicians, patients and families.
Self‑directed but functions well in a team environment.
Excellent clinical skills—assessment, diagnosis, treatment, prescribing specifically for the chronically ill population.
Can function independently, demonstrating critical thinking, organization and prioritization skills.
