Summary Serves as a patient advocate, liaison, and adviser/educator to assist patients with navigating the continuum of care.

Responsibilities

Assists patients and families with resolving financial, psychosocial, functional, and administrative issues by advising options and referring to appropriate resources.

Intervenes as appropriate to advocate for patient and family.

May collect and assess data to identify quality trends or sources of risks to patients and employees.

Collaborates with multidisciplinary team and committees with respect to quality outcomes.

Continuously reviews and evaluates patient progress as outlined within the plan of care.

Communicates plan of care to patient and family, solicits concerns, questions, and issues for resolution.

Provides emotional support, counseling, clinical education and expert guidance to patients and families to promote understanding and meaningful participation in the healthcare process and personal decision‑making.

Serves as a liaison between patient and clinical staff, administration, physicians, and community/external resources to coordinate/maximize resources and identify/resolve barriers to the plan of care.

Education and Certification Requirements

Accredited nursing program (required)

BLS American Heart E‑Card (BLS AHA ECARD)

BLS American Heart RQI E-Card (BLS AHA‑RQI ECARD)

BLS Cert American Heart non‑ecard (BLS AHA)

BLS Certification Grace (BLS GRACE) – employee grace period for essential credential (GRACE)

BLS Cert Red Cross (BLS RC)

Registered Nurse Compact License (RN Compact) – multistate

Registered Nurse License (RN) – State of Florida

Additional – Graduate of an accredited registered/professional nursing program or MSW degree; if RN, licensed; Master’s in Social Work preferred.

Additional Job Information Complexity of work: requires critical thinking, effective communication, decisive judgment, and ability to work with minimal supervision. Must handle stressful environment and take appropriate action.

Required work experience: one (1) year in related field. If RN, must be licensed. Master’s in Social Work (MSW) preferred. (RT) and certified in Radiation Therapy by ARRT required if working in Radiation Oncology.
