Practice and promote the agency’s mission statement and guiding principles in every interaction. Provide ongoing support to the people that live in the Community Living Services program to ensure that a high quality of life is maintained. Provide support in the areas of, for example, self‑care routines, health, safety, household chores, socialization, and overall wellbeing.

Responsibilities

Provide ongoing guidance and support to individuals with intellectual and other disabilities to ensure active participation in every aspect of their lives, offering only the level of support necessary to promote independence and engagement.

Assist individuals in becoming active members of the community and in building upon natural supports and relationships.

Maintain a safe and clean home by supporting individuals with household chores and, when needed, participating in those chores.

Follow all treatment plans and employ all necessary adaptive and safety equipment to promote independence, health, safety, and well‑being.

Support individuals in meal preparation by teaching concepts such as diet adherence, portion control, and cooking skills.

Assist with medication administration in accordance with AMAP policy and procedures.

Implement and document all treatment plans and agency‑related materials (e.g., residential habilitation goals, medical case notes, communication logs) to support individuals in achieving independence and selected valued outcomes.

Adhere to overtime, scheduling, and minimum staffing policies to ensure quality support each day.

Participate in the inter‑disciplinary team by attending meetings, discussing progress, and sharing information.

Provide transportation for medical appointments, shopping, recreational outings, and other transportation needs.

Mentor new Direct Support Staff by demonstrating supportive routines and reviewing training materials.

Participate in House Meetings and maintain all certification and training expectations.

Perform any other duties as requested by the supervisor.

Qualifications

High School Diploma or equivalent preferred.

Valid NYS driver’s license and maintained qualified driver status throughout employment.

Ability to communicate and understand written and oral directives/information.

Additional Requirements Physical Aspects may include but are not limited to lifting and transferring of individuals (children and/or adults). Inability to meet the physical demands of the position may impact employment.

Compliance This position requires regular and substantial unsupervised or unrestricted physical contact with people receiving services and, therefore, is subject to successful completion of fingerprint/criminal background check.

Training on Personal Outcome Measures (POM) is required prior to completion of evaluations and assessments.
